Change of Notary’s Name

If your name changes during your commission period, either due to marriage or court order, you need to submit a name change form, which can be located at http://www.sos.ca.gov/business/notary/forms/notary_name_chg_app_7-03.pdf

You will be sent an amended commission, and you will need to file a new oath of office and an amendment to your bond within 30 days from the date of the amended commission with the county clerk in the county of your principal place of business.  Your commission number and expiration date remain the same. 

You may continue your notarial duties using your old name until the amended commission takes effect.  Also, within 30 days of filing your new oath and amended bond, you must obtain a new notary seal reflecting your new name.

If you willfully fail to notify the Secretary of State of your name change, you may be charged with an infraction, which carries a fine of up to $500.
