Durable Power of Attorney for Health Care
This document
requires either the signatures of two witnesses to the signing (one
of whom must have no relation to the signer) or be acknowledged
before a notary public. If you are acknowledging a Durable Power of
Attorney for Health Care, all of the normal notarial procedures
apply. You must verify the signer’s identity and complete the
appropriate notarial certificate and journal entry and obtain a
right thumbprint of the signer.
